Montage Health Foundation Scholarship Fund

Scholarship Guidelines:
Amount:

• up to $3,000 for community college and vocational education applicants
• up to $6,000 for four-year university applicants
• up to $10,000 for graduate/medical school applicants
  - Recipients are eligible to renew for up to 3 years
  - Applicants must pursue a degree/career in the healthcare industry (including laboratory work, nursing, radiology, etc.)

Scholarship Criteria:
- Current employees of Montage Health are ineligible. Dependents of Montage Health employees are eligible to apply.
- High School seniors are not eligible.

The applicants must be:
- The applicant must be a Monterey County resident
- Currently enrolled in a post-secondary institution pursuing a career in healthcare industry
- Demonstrated financial need
- Demonstrated interest in completing a Bachelor’s degree, Graduate degree or Vocational School certification

To claim the award funds, the winning students must:
 Provide proof of enrollment in an accredited community college, university or vocational education institution pursuing an associate, bachelor’s, graduate degree or vocational training certificate
 As a full-time student
 by Fall (or) Spring semester after receiving the award
 Send the Claim Form provided by CFMC along with official proof of his or her enrollment to the Community Foundation
